Parents Melissa and her husband are devastated after their eight-year-old son, Andrew Sheldrick, died due to a critical medication error at a compounding pharmacy. Andrew suffered from parasomnia, a REM sleep disorder, and was prescribed Tryptophan to aid his sleep. However, Floradale Medical Pharmacy Ltd. in Mississauga mistakenly dispensed Baclofen, a potent muscle relaxant, instead.
A Fatal Medication Mix-Up
The dose of Baclofen administered was nearly three times the toxic level for an adult, leading to Andrew's tragic death. His parents discovered him unresponsive the morning after he took the medication, unaware of the deadly substitution. The packaging of the incorrect medication was identical to the original, offering no immediate clues to the error.
Legal Action and Call for Accountability
Following the loss, the Sheldrick family has filed a multi-million dollar lawsuit against Floradale Medical Pharmacy Ltd., its owner Amit Shah, and the responsible pharmacist or technician. The lawsuit alleges negligence, including dispensing errors, poor record-keeping, and violations of pharmacy regulations.
Advocating for Systemic Change
Beyond seeking justice for Andrew, Melissa Sheldrick is campaigning for legislative reform. She advocates for mandatory public reporting of all prescription errors by pharmacies. Currently, only Nova Scotia requires such reporting to ISMP Canada, while Ontario and other provinces do not.
Melissa believes that increased transparency and accountability are crucial to preventing future tragedies. Floradale Medical Pharmacy Ltd. has declined to comment on the ongoing legal proceedings.
